For those who have been with us for a while, you may recall that there has been a bit of a reversal in thinking over the past few years regarding the cause of peripheral neuropathy (PN).
Back when we were in school, we were taught that the cause of peripheral neuropathy, especially diabetic peripheral neuropathy, was an occlusion to the tiny artery that fed the nerve called the vasa nervorum. A vascular (circulation) problem.
The problem with that early theory was that it failed to explain several observations about our patients. Why, for instance could we see robust circulation in the face of PN?
It just didn’t completely answer all the questions. Toward a better answer, science more recently adopted a metabolic theory of neuropathy and nerve dysfunction that answered more… but not all… of those nagging questions. In this theory, oxidative by-products, toxins really, from diabetes, or many other causes, directly affected the nerve function resulting in pain, burning, numbness and tingling, especially in the feet and hands.
Just recently, a group of researchers in Germany may have linked these two theories into a single explanation. It explains a lot.
They suggest that the metabolic process, so well established already, may actually cause the micro-vascular occlusion observed in clinical settings and the laboratory.
